Youth Transition Coordinator
8 hours per day / Limited Duration Position
Anticipated Start Date: ASAP
End Date: 6-13-2024
Position Serves the Molalla High School and District Office.
ESSENTIAL REQUIREMENTS: To perform this job successful and individual must be able to:
- Identify and refer students for OVRS services
- Secure school documentation of disability and relevant anecdotal information to assist YTP team in determining eligibility for OVRS services
- Assist students in acquiring transportation to keep appointments
- Coordinate with OVRS counselors and student to develop any needed additional disability assessment or career exploration
- Identify transition and career-skill readiness deficits that need to be in the
- Coordinate IEP and IPE transition
- In collaboration with the OVRS counselor, provide a variety of transition services, such as: development of work experiences, job shadows, and career exploration activities, provide job coaching; and develop paid employment consistent with the vocational goal of the
- Advocate for students to ensure that they have access to District training opportunities and resources that are needed in order for the student to achieve a successful school-to-career
- Provide individualized and group activities that are beyond the training or resources that are available to the general student Examples job clubs, community-based work experiences, work skill development, job coaching, community mobility training, selection or enrollment in post secondary training, arranging for long-term employment supports, etc.
- Take lead responsibility assisting the student to ensure that the activities of IPE are carried
- May take a lead with the student to develop the content of the IPE (including assisting the student in the identification of vocational goals).
- Provide follow-up services following the student's completion or termination of campus based secondary schooling activities, to ensure success in post-secondary training or
- Collect and report data to OVRS and the YTP technical assistance provider (University of Oregon) to document student progress toward completion of activities related to IEP and Individualized Plan for
- Meet with OVRS counselors as requested to provide updated information on student progress and status for completion of secondary school, employment, or post secondary education
- Report to OVRS counselor any changes in status of student that impact successful completion of IEP or
- Attending training and meetings provided by the OVRS-designated technical assistance provider for YTPs statewide, which is the University of
- Coordinate with outside agencies to meet the needs of the
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Coordinate with Transition and Life Skills' teachers
- Coordinate with C TEC staff @ Clackamas Community College
- Prepare and distribute letters, flyers, brochures and forms for the YTP and C TEC programs
- Consult building principals and secretaries about programs affecting their buildings
- Proven ability to develop jobs in the local community
- Proven ability to work successfully with outside agencies
Reports directly to the Special Services Director
CERTIFICATES, LICENSES, REGISTRATIONS, BONDING, AND/OR TESTING REQUIRED:
- Possess and maintain a valid First Aid card if required
- Criminal Justice Fingerprint Clearance
